Segui il tuo cuore, il film diretto da Burr Steers e tratto dal romanzo "Ho sognato di te" di Ben Sherwood, vede protagonista Charlie St.Cloud (Zac Efron), un giovane neodiplomato che condivide una forte passione per la barca a vela con il fratellino minore Sam (Charlie Tahan), a cui è molto legato.
Quando Charlie ottiene anche una borsa di studio per il college che potrà permettergli di proseguire con la sua carriera da velista, il suo brillante futuro viene stroncato da un terribile incidente stradale a cui lui miracolosamente riesce a sopravvivere, al contrario del piccolo Sam che non riesce a salvarsi.
Così Charlie, distrutto dalla sua perdita, abbandona il sogno della barca a vela e rinuncia al college decidendo di lavorare come custode nel cimitero in cui riposa il fratellino. Charlie, non riuscendo ad accettare la morte di Sam, allontana chiunque dalla sua vita e continua ad avere visioni del fratello con cui si incontra ogni sera al tramonto per allenarsi a baseball e la cui presenza svanisce al calar del giorno. Ma quando la vecchia compagna di liceo Tess (Amanda Crew) ritorna improvvisamente in città e rientra a far parte della vita di Charlie, il ragazzo dovrà decidere se rinunciare all'amore o se trovare invece il coraggio di liberarsi del suo passato e andare avanti una volta per tutte, lasciando andare Sam.
