Il "nido del cuculo" (cuckoo's nest) al quale si fa riferimento nel titolo è una delle molte espressioni del gergo statunitense che indicano il manicomio. Esiste anche una filastrocca che contiene il seguente verso: Three geese in a flock, one flew East, one flew West, one flew over the cuckoo's nest ("Uno stormo di tre oche, una volò ad est, una volò ad ovest, una volò sul nido del cuculo" . Di questa filastrocca non c'è traccia nel film, mentre costituisce uno degli elementi essenziali del romanzo di Kesey.
Qualcuno volò sul nido del cuculo (One Flew Over the Cuckoo's Nest) è un film del 1975 diretto da Miloš Forman.
È tratto dal romanzo omonimo di Ken Kesey, pubblicato nel 1962 e tradotto in italiano nel 1976 da Rizzoli Editore. L'autore scrisse il libro in seguito alla propria esperienza da volontario all'interno del Veterans Administration Hospital di Palo Alto, in California.
Ha segnato la storia del cinema nella trattazione innovativa di un argomento molto delicato come il disagio presente negli ospedali psichiatrici, denunciando il trattamento inumano cui venivano sottoposti i pazienti ospitati nelle strutture ospedaliere statali, verso i quali vigeva un atteggiamento discriminatorio, alimentato dalla paura dell'aggressività che caratterizza in qualche caso la malattia mentale.
È uno dei tre film nella storia del cinema (insieme a Accadde una notte di Frank Capra e Il silenzio degli innocenti di Jonathan Demme) ad aver vinto tutti e cinque gli Oscar principali (miglior film, miglior regista, miglior attore, miglior attrice, migliore sceneggiatura non originale).
Nel 1993 è stato scelto per la conservazione nel National Film Registry della Biblioteca del Congresso degli Stati Uniti. Nel 1998 l'American Film Institute l'ha inserito al ventesimo posto della classifica dei migliori cento film statunitensi di tutti i tempi, mentre dieci anni dopo, nella lista aggiornata, è sceso al trentatreesimo posto.

Randle Patrick McMurphy (Jack Nicholson) arriva nel 1963 all'ospedale psichiatrico di Salem, e viene accolto dal Dottor John Spivey (Dean R. Brooks), che gli spiega a quali trattamenti verrà sottoposto per capire la natura della sua malattia mentale. Durante il periodo di osservazione, assume un comportamento insubordinato e istiga alla ribellione gli altri pazienti: rifiuta le medicine, prende in giro gli infermieri e i degenti, si impossessa dell'autobus dell'ospedale e con altri compagni ruba una barca da pesca, fingendo di far parte di un gruppo di medici della struttura. Sotto la sua guida e grazie al suo esempio, gli altri pazienti iniziano a ribellarsi ai trattamenti vessatori degli infermieri e soprattutto della temibile caporeparto, Mildred Ratched (Louise Fletcher). Randal stringe amicizia con gli altri degenti, in particolare con Billy (Brad Dourif), ragazzo timido e balbuziente, e con Bromden (Will Sampson), sordomuto e dalla corporatura imponente, e cerca di aiutarli a comprendere che sono persone degne di rispetto e di essere trattate come tali, ma quando capirà che è impossibile cambiare realmente le cose, organizzerà la fuga.
