Ray Gaines (Dwaine Johnson) è un pilota di elicotteri della squadra di soccorso dei vigili del fuoco. L'uomo sta facendo i conti con un triste passato e con un difficile divorzio da sua moglie Emma (Carla Gugino). Nel frattempo, il sismologo Lawrence Hayes e il suo collega Kim Park sono alla diga di Hoover a testare un nuovo modello di previsione dei terremoti. All'improvviso si scatena un terremoto di magnitudo 7,1 che fa crollare la diga: Hayes scopre che l'intera faglia di San Andreas si sta spostando e causerà presto una serie di gravi sismi in grado di distruggere tutte le città lungo la sua linea.
Come gli scienziati avevano predetto, un terremoto di magnitudo 9,1 devasta Los Angeles. Nello scenario apocalittico, Ray riesce a salvare sua moglie e insieme si dirigono a San Francisco per trovare e recuperare la loro figlia Blake (Alexandra Daddario), rimasta intrappolata nell'ufficio del compagno di Emma, Daniel Riddick (Ioan Gruffudd), abbandonata lì da quest’ultimo dopo l'inizio del sisma.
Ben Taylor, un ingegnere britannico in cerca di lavoro, e suo fratello minore Ollie riescono però a salvare la ragazza e insieme raggiungono Chinatown. Dopo aver sentito la figlia ed essersi dati appuntamento in un luogo apparentemente sicuro, Ray ed Emma tentano di raggiungere San Francisco. Tuttavia, superare la devastazione della natura risulterà un vera impresa. Dovranno fare presto, perché il più grande terremoto mai registrato nella storia di magnitudo 9,6 sta per colpire la faglia distruggendo tutto ciò che incontrerà…
