Primo atto
Uno dei personaggi, Francis, racconta in analessi una storia sinistra a un interlocutore, un uomo anziano seduto di fianco a lui: l'analessi ritorna al 1830, nel piccolo paese di Holstenwall in Germania, in cui un signore poco raccomandabile, di nome Caligari,[3] giunge alla fiera del paese per presentare il suo sonnambulo, Cesare, che tiene sotto ipnosi in una cassa da morto. Il dottor Caligari sostiene che il sonnambulo, una volta svegliato, sia in grado di conoscere il passato e di predire il futuro.

Secondo atto
Con l'arrivo del dottor Caligari nel paese, cominciano ad avvenire morti sospette: la prima vittima è il segretario della fiera, che si era dimostrato estremamente scortese e irriverente nei confronti di Caligari.
Il giorno dopo, l'amico del narratore, Alan, è il primo a chiedere una predizione: i due uomini condividono l'amore per Jane, bellissima fanciulla che essi si contendono pacificamente. Cesare predice ad Alan che morirà entro il mattino seguente, cosa che effettivamente si avvera: l'uomo viene pugnalato nel proprio letto.
Terzo atto
Le indagini si concentrano da subito su Cesare, ma il dottore dimostra che questi è sempre rimasto con lui a dormire nella sua bara: viene dunque arrestato un bandito, colto in flagrante nell'atto di pugnalare una vecchietta.
Quarto atto
Il criminale arrestato confessa di aver tentato di uccidere la signora con le stesse modalità degli altri due omicidi allo scopo di scaricare la propria colpa sul misterioso assassino, ma di non averli commessi. Intanto Jane, in cerca del padre, si imbatte nel dottor Caligari: questi, dopo averla spaventata, ordina a Cesare di ucciderla nella notte. Quando Cesare è sul punto di farlo, rimane però estasiato dalla bellezza eterea della giovane, quindi la rapisce. Venendo inseguito dalla folla, sfinito, è costretto ad abbandonarla, e dopo aver percorso un ulteriore tratto, non visto, cade a terra morto. Una volta in salvo, Jane rivela che il rapitore era Cesare.
Quinto atto
Francis, che era rimasto a osservare di nascosto dalla finestra dell'abitazione del dottor Caligari, rimanendo sconcertato nel vedere che Cesare continuasse a dormire nella sua bara con il dottore accanto a lui, chiede aiuto alla polizia. Insieme si recano presso l'abitazione del dottor Caligari, dove scoprono che il Cesare che vedono dalla finestra è in realtà solo un fantoccio che il dottor Caligari utilizza per coprire i propri movimenti. Smascherato e inseguito dalle forze dell'ordine, il dott. Caligari si rifugia presso un manicomio di cui è lui stesso il direttore.
Qui Francis svela il mistero, grazie al ritrovamento del diario segreto del dottore: in preda all'insaziabile desiderio di ricerca nel campo del sonnambulismo, il dottore aveva scoperto come nel 1703 fosse vissuto uno psicologo, di nome Caligari, che girando di città in città nell'Italia settentrionale si era servito di un paziente affetto da sonnambulismo come assassino, contro la volontà stessa di quest'ultimo: volendo imitarlo, e approfittando dell'arrivo al manicomio di Cesare, affetto da sonnambulismo, il dottore aveva avviato con lui i propri esperimenti, riuscendo a comandarlo a proprio piacimento e convincendosi sempre di più di essere lui stesso il dottor Caligari.
Sesto atto
Viene ritrovato il cadavere di Cesare: il direttore del manicomio, vistosi scoperto e ormai incriminato, aggredisce Francis e gli infermieri, ma gli viene infilata una camicia di forza e viene rinchiuso in una cella di isolamento.
Mentre il racconto termina con la detenzione forzata del dottore nel suo stesso manicomio, si conclude la lunga analessi, alla panchina dove sono seduti Francis e il suo ascoltatore; ma un finale a sorpresa lascia intendere che forse il racconto di Francis non è stato altro che frutto della sua fantasia: Francis, Jane e Cesare sono infatti pazienti del manicomio menzionato nel racconto e la persona che Francis sostiene essere il dottor Caligari altri non è che l'attuale direttore dello stesso istituto correttivo; nel vederlo, Francis dà in escandescenze, e prima che possa aggredirlo gli viene messa la camicia di forza.
La pellicola si conclude con Francis rinchiuso in cella di isolamento. Il direttore del manicomio afferma che saprà come trattare Francis, ora che è emerso l'oggetto della sua ossessione: il dottor Caligari.
As Francis sits on a bench with an older man who complains that spirits have driven him away from his family and home, a dazed woman named Jane passes them. Francis explains she is his "fiancée" and that they have suffered a great ordeal. Most of the rest of the film is a flashback of Francis's story, which takes place in Holstenwall, a shadowy village of twisted buildings and spiraling streets. Francis and his friend Alan who are good-naturedly competing for Jane's affections, plan to visit the town fair. Meanwhile, a mysterious man named Dr. Caligari seeks a permit from the rude town clerk to present a spectacle at the fair, which features a somnambulist named Cesare. The clerk mocks and berates Caligari, but ultimately approves the permit. That night, the clerk is found stabbed to death in his bed.
The next morning, Francis and Alan visit Caligari's spectacle, where he opens a coffin-like box to reveal the sleeping Cesare. On Caligari's order, Cesare awakens and answers questions from the audience. Despite Francis's protests, Alan asks, "How long shall I live?" To Alan's horror, Cesare answers, "The time is short. You die at dawn!" Later that night, a figure breaks into Alan's home and stabs him to death in his bed. A grief-stricken Francis investigates Alan's murder with help from Jane and her father, Dr. Olsen, who obtains police authorization to investigate the somnambulist. That night, the police apprehend a criminal in possession of a knife who is caught attempting to murder an elderly woman. When questioned by Francis and Dr. Olsen, the criminal confesses he tried to kill the elderly woman, but denies any part in the two previous deaths; he was merely taking advantage of the situation to divert blame away from himself.
At night, Francis spies on Caligari and observes what appears to be Cesare sleeping in his box. However, the real Cesare sneaks into Jane's home as she sleeps. He raises a knife to stab her, but instead abducts her after a struggle, dragging her through the window onto the street. Chased by an angry mob, Cesare eventually drops Jane and flees; he soon collapses and dies. Francis confirms that the criminal who confessed to the elderly woman's murder is still locked away and could not have been Jane's attacker. Francis and the police investigate Caligari's sideshow and realize that the "Cesare" sleeping in the box is only a dummy. Caligari escapes in the confusion. Francis follows and sees Caligari go through the entrance of an insane asylum.
Upon further investigation, Francis is shocked to learn that Caligari is the asylum's director. With help from the asylum staff, Francis studies the director's records and diary while the director is sleeping. The writings reveal his obsession with the story of an 11th-century monk named Caligari, who used a somnambulist named Cesare to commit murders in northern Italian towns. The director, attempting to understand the earlier Caligari, experiments on a somnambulist admitted to the asylum, who becomes his Cesare. The asylum director screams, "I must become Caligari!" Francis and the doctors call the police to Caligari's office, where they show him Cesare's corpse. Caligari then attacks one of the staff. He is subdued, restrained in a straitjacket, and becomes an inmate in his own asylum.
The narrative returns to the present, where Francis concludes his story. In a twist ending, Francis is depicted as an asylum inmate. Jane and Cesare are patients as well; Jane believes she is a queen, while Cesare is not a somnambulist but awake, quiet, and not visibly dangerous. The man Francis refers to as "Dr. Caligari" is the asylum director. Francis attacks him and is restrained in a straitjacket, then placed in the same cell where Caligari was confined in Francis's story. The asylum director announces that, now that he understands Francis's delusion, he is confident he can cure him.
