La storia inizia nel 1919 nella povertà della Birmingham del dopoguerra dove tutti lottano per sopravvivere ad un periodo particolarmente difficile dal punto di vista storico ed economico. La storia è ambientata nel quartiere di Small Heath e si focalizza particolarmente sulla famiglia Shelby, cui capofamiglia, è anche il boss della gang detta "Peaky Blinders", dall'usanza di nascondere una lametta nel risvolto dei cappelli, la quale poteva essere utilizzata anche come arma.
La leadership di Tommy Shelby è messa a dura prova dall'arrivo in città da Belfast dello spietato poliziotto Campbell, deciso a ripulire il marcio e la delinquenza a tutti i costi, e da una donna, Grace Burgess, dal passato misterioso.
