A shuddering exorcism spins out of control and claims the life of a young girl. Months later, while working the night shift at the hospital morgue, Megan Reed receives a disfigured corpse. Alone and locked in the corridors of the cold basement, she starts to experience hair-raising visions, which lead her to suspect that the dead body delivered that night is possessed by a ruthless evil entity.
Una giovane ragazza, Hannah Grace, viene esorcizzata da un paio di sacerdoti, alla presenza di suo padre, perché posseduta da spiriti maligni. L'esito dell'esorcismo è tragico: i sacerdoti vengono uccisi e il padre, a sua volta, uccide Hannah, disperato. Tre mesi dopo, Megan Reed, ex poliziotta che ha lasciato il servizio per problemi, prende servizio per il turno di notte alla morgue di un grande ospedale, il Boston Metro Hospital. Il dottor Lewis che la conduce al posto di lavoro la avvisa che parecchia gente impressionabile ha dovuto lasciarlo perché non lo reggeva, ma Megan assicura che per lei quando uno muore, muore e basta. Fine della storia. Perciò lavorare a una morgue per lei non sarà un problema, anche se dovrà restarvi praticamente da sola per tutta la notte.
La prima notte di lavoro, Megan riceve subito un cadavere: svolge il suo lavoro e tutto va bene. La notte successiva, Megan riceve un altro cadavere, quello di una giovane donna. Non si accorge, però, che uno strano tipo che prima le aveva chiesto di entrare si intrufola dentro la morgue.
La ragazza è stata uccisa e mutilata da un ignoto assassino. La sua carta d'identità rivela che si tratta di Hannah Grace. Subito cominciano ad accadere cose strane e Megan si ritrova ad affrontare qualcosa di terribile.
